Lab Orders – if you need a copy of your lab order, you can find it in your patient portal. We are unable to send any orders via text or email. If you need assistance logging into your portal, please let us know.
Cancelation and No-Show Policy – If you cannot make your appointment, please let us know within 48 hours. This allows us to free up your time for someone else, while allowing you to avoid a $50 no-show fee for urology appointments and a $100 no-show fee for physical therapy appointments.
